Position: Service Coordinator

Service Coordinator positions are available in Hillside. Coordinators work with families entering the Illinois Early Intervention system. Experience in early intervention service coordination is desirable. A Bachelor degree in a human service field is required, along with a valid driver’s license and proof of vehicle insurance. Program: DD Service Coordination
Position: QMRP, Individual Service Coordinator

Positions are available in Homewood and Hillside to provide service facilitation and coordination to individuals with a developmental disability. Individual Service

Coordinators conduct visits to the person’s day/residential programs to assess service satisfaction. Individual Service Coordinators may also determine individuals eligible for services and link them with necessary services. This position requires one year of experience working with persons with developmental disabilities. Applicants must be organized and able to travel locally.
